Emily M. Fackelman, MSW

Emily M. Fackelman, MSW helps develop and implement short- and long-term care plans and advocates for our clients throughout the process. Emily prepares families and attends the in-person and virtual assessments conducted by various agencies to help clients secure the proper Medicaid home care hours and services.

Additionally, Emily and the Social Work team educate and support clients and their families by partnering with home care agencies and other related providers to coordinate home health aides and various other services that positively impact our client’s lives.

Futterman, Lanza & Pasculli also represents clients with both intern and external appeals with Managed Long Term Care (MLTC) plans, including helping clients who were previously denied before coming to our firm.

Prior to joining the law firm, Emily was a Social Worker at White Oaks Rehabilitation & Nursing Center and before that position, she was a Social Worker at several other Rehabilitation Centers. These positions expanded her knowledge base and her vast local contacts in the senior focused professional community as she worked with many seniors and their families and built strong relationships based on trust to accomplish goals.

In her most recent role, Emily completed multifaceted assessments with all new admissions as well as conducted quarterly assessments on overall care, worked with discharge planning and medical records. She was instrumental in conducting smooth transitions for the residents to and from Assisted Livings and she educated and assisted with advanced directives.

Emily takes a collaborative team approach and is a calming voice with our clients who may be in some type of crisis mode. Her belief in collaboration combined with the strength of her background and her commitment to the clients is a sure recipe for success.

Emily works primarily in our Smithtown office while visiting the Garden City and Bay Shore offices as well as being in the community as needed.

Emily Fackelman received her Master of Social Work, Advanced Standing 1 from State University of New York at Stony Brook with a focus on Integrated Health and Aging. She holds her Bachelor of Science, Social Work from State University of New York at Fredonia.
